Job summary

Talented Medical Solutions is seeking a Licensed Practical Nurse for a long-term care facility in Wisconsin. You will deliver nursing care under supervision, administer medications, monitor residents, and supervise CNAs on your unit.

Responsibilities include completing accurate documentation, communicating changes to the care team, and ensuring infection control and safety standards are followed. Active LPN/LVN licensure and BLS are required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• BLS certification required.
• Prior experience in a long-term care or skilled nursing setting preferred.

Responsibilities

• Administer medications and treatments per physician orders and facility policy.
• Perform resident assessments and monitor changes in physical, cognitive or behavioral status.
• Supervise and delegate appropriately to Certified Nursing Assistants on the assigned unit.
• Complete accurate and timely documentation in the resident chart and MAR.
• Communicate changes in resident condition to the RN, physician and family as appropriate.
• Follow infection control, safety and regulatory requirements governing long-term care practice.
