Qualifications

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in New York.
• BLS Certification from the American Heart Association.

Responsibilities

• Administer medications timely and document in the patient record.
• Perform treatments per physician orders and facility protocol.
• Support CNA team and supervise care plans with accurate documentation.
• Communicate patient status to charge nurse/supervisor and physicians as needed.
• Complete MDS documentation and related observations timely.
• Direct CNA team to deliver individualized care per care plans.
