Job summary

Qualifications
- Graduate from an accredited School of Practical Nursing.
- Current LPN licensure in State of New York.
- BLS Certification from the American Heart Association (AHA).
- Preferred 1 year experience.
Responsibilities
- Administers medications accurately and documents progress per orders and facility protocol.
- Performs treatments per physician orders; documents progress.
- Maintains complete resident documentation using the EMR; initiates care pathway tools.
- Transmits reports verbally and in writing per departmental protocols.
- Monitors residents and reports changes to the charge nurse/supervisor/physician promptly.
- Completes MAR pre-reviews per facility protocols.
- Completes required MDS documentation and related observations timely.
- Supports CNA team with direct ADL care of residents.
