Qualifications

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in New York State.
• BLS certification by the American Heart Association.

Responsibilities

• Responsible for timely, accurate medication administration and documentation.
• Performs treatments and documents progress per orders and protocol.
• Maintains concise resident care documentation in the medical record.
• Initiates appropriate care pathway tools as needed.
• Receives/gives reports to staff per departmental protocols.
• Monitors residents and communicates status to charge nurse or physicians promptly.
• Completes medication administration record reviews and related observations.
• Completes MDS documentation and related observations timely.
• Supports CNAs and assists in direct ADL care per care plans.
• Directs CNAs to complete quality personal care per plans.

Education

Graduate from an accredited School of Practical Nursing

Current LPN licensure in New York State

BLS Certification - American Heart Association
