Job summary

Sweetwater Care seeks an LVN/LPN to provide nursing care within scope of practice and ensure patient needs are met per standards, physician orders, and policies.

Requirements include active LVN/LPN license, BLS/CPR, and experience in long-term care; PCC charting experience is preferred. Management experience is a plus but not required. This role involves charting, rounds with physicians, and adherence to regulations.

Qualifications

• Experience in long-term care nursing.
• Proficiency with PointClickCare charting.
• Licensed LVN/LPN in practicing state.
• BLS/CPR certification current or willingness to acquire.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
