Job summary

Sweetwater Care is seeking an LVN/LPN in Colorado to provide nursing care within scope of practice, aligning with physician orders and center policies to meet patient needs. Thorough documentation and proactive communication are essential to quality patient care.

The role requires long-term care experience, PointClickCare charting skills, an active LVN/LPN license in Colorado, and BLS/CPR certification. Nursing management experience is a plus but not required.

Qualifications

• Long-term care experience required.
• PointClickCare charting experience.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Nursing management experience is preferred but not required.

Responsibilities

• Chart and incident reporting per established procedures.
• Narcotics counting, pharmacy orders, and restraint reduction per company policies.
• Document changes in patient conditions and notify applicable departments.
• Utilize the Quality Life Manual for patient placement and monitoring.
• Document room changes, LOA, new admits, or discharges and notify relevant staff.
• Mornings/evening rounds with physicians when necessary.
