Job summary

American Baptist Homes of the Midwest is seeking a Licensed Practical Nurse to provide hands-on nursing care in a long-term care setting at Tudor Oaks in Muskego, WI. You will support residents'' physical, mental, and social well-being and help build/update care plans through assessments and evaluations.

The role requires an active LPN license in Wisconsin (or transferable from another state) and at least 1 year of LTC/healthcare experience.

Qualifications

• Active LPN license in Wisconsin or transfer-able out-of-state license.
• Graduate of an accredited school of nursing.
• At least 1 year of LPN experience in healthcare or long-term care.
• Able to read, write, and speak English for medication administration and care documentation.

Responsibilities

• Provide hands-on nursing care to residents in a long-term care setting.
• Support residents' physical, mental, and social well-being.
• Help build and update care plans through assessments and evaluations.
• Carry out nursing interventions based on each resident's care plan.
• Work closely with residents, families, and the care team, including a clear handoff between shifts.
