Trinity Homes in Minot, ND seeks an LPN to provide direct nursing care under the supervision of a registered nurse. You will support individualized care plans, monitor residents, and participate in ongoing assessments to promote safety, comfort, and quality of life.

Collaborating with CNAs, providers, and the interdisciplinary team, you will document care activities clearly and ensure continuity across shifts while maintaining a compassionate, therapeutic environment.

Qualifications

• Current LPN license and eligibility to practice.
• Ability to collaborate with RNs, CNAs, and providers.
• Strong documentation and communication skills.

Responsabilités

• Assist the RN with assessments and ongoing resident monitoring.
• Administer medications and treatments per care plans.
• Document assessments and interventions in the electronic medical record.
• Communicate changes to the RN, provider, and family and update care plans.
• Supervise CNA staff during daily care activities.
• Maintain infection control and a safe, homelike environment.
