Job summary

Adoration Health seeks a Private Duty Licensed Practical Nurse in Knoxville, TN, to deliver patient-centered in-home care. The role emphasizes independence with team support, 1:1 client contact, and adherence to individualized care plans in a supportive home health environment.

Applicants should have an LPN/LPT/LVN license, CPR certification, and at least 1 year of nursing experience, with home health exposure preferred. A valid driver’s license and reliable vehicle are required.

Responsibilities

• Provide 1:1 client care according to the plan of care, maintaining dignity and independence.

• Document nursing actions and communicate with family, case manager, and physicians; report changes to the care team.
• Coordinate or order supplies, medications, and feeding supplies to ensure adequate stock.
• Ensure patient needs are met as per physician instructions.

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
