Adoration Health, an affiliate of BrightSpring Health Services, seeks a Private Duty LPN to join our Chattanooga, TN team for 1:1 patient care in the home setting.

The role offers PRN, flexible scheduling, and opportunities for professional growth within a supportive organization.

Competenze

• LPN/LPT/LVN degree from an accredited nursing program with active license in the practice state.
• Valid driver’s license and automobile insurance.
• CPR certification.
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Mansioni

• Provide 1:1 client care per the plan of care while maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as required and document results.
• Document nursing actions and communicate changes to family, case manager, and physicians.
• Coordinate or order supplies, medications, and feedings to ensure adequate stock.
• Ensure patient needs align with physician instructions.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
