Adoration Health is seeking a Private Duty LPN to provide patient-centered, 1:1 home health care in Jackson, TN. The role offers part-time daytime shifts with potential for full-time work through another company.

You will administer medications and treatments per care plans, document nursing actions, and coordinate supplies while maintaining patient dignity and independence. A valid LPN license and CPR certification are required.

Qualifications

• LPN/LPT/LVN degree with current unrestricted registration and license in the state of practice
• Valid driver’s license with acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home health care experience preferred

Responsabilités

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Schedule: Part-Time, Days (potential to Full-Time working in connection with another Company)

Patient Needs: Skilled Nursing

Shift Details

• 1 - 2 day shifts a week
• 9:00am - 9:00pm
• Week 1: Tuesday, Saturday, Sunday
• Week 2: Tuesday, Thursday, Friday
