Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Private Duty LPN for Jackson, TN. You will provide patient-centered, 1:1 home health care following each client’s plan of care with independence and team support.

The role offers full-time hours (days) with a focus on stability and growth within a growing company; current benefits include comprehensive insurance options and other perks.

Formación

• LPN/LPT/LVN license in state of practice with active registration.
• CPR certification required.
• Valid driver's license and proof of car insurance.

Responsabilidades

• Provide 1:1 client care according to care plan while maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed per plan and document results.
• Document nursing actions, communicate with family/case manager/physicians and note changes in patient condition.
• Coordinate or order supplies, medications and feedings to ensure availability.
• Ensure patient needs are met per physician's instructions.

Schedule: Full-Time; Days

• Alternating week schedule


• Week 1: Monday, Tuesday, Thursday, Friday 7:30am - 5:30pm


• Week 2: Monday, Tuesday, Saturday, Sunday 7:30am - 5:30pm

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence


• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results


• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs


• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand


• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

Qualifications:

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice


• Valid driver's license, acceptable driving record and proof of car insurance


• CPR professional certification


• 1+ years of nursing experience preferred


• Home Health care experience preferred
