Adoration Health in Brownsville, TN is seeking a Private Duty LPN to provide expert, one-on-one in-home care under a patient’s plan of care. This full-time overnight role covers the Jackson, TN area with Sundays through Thursdays 9:00pm–7:00am, and offers a supportive team and opportunities for advancement.

The position includes comprehensive benefits, mileage reimbursement, and PTO, with a focus on patient dignity and independence in a home health setting.

Formación

• LPN/LPT/LVN degree with current state license.
• Valid driver’s license with proof of insurance.
• CPR certification (professional).
• 1+ years of nursing experience preferred.
• Home Health care experience preferred.

Responsabilidades

• Provide 1:1 care following the client’s plan of care at all times.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate supplies, medications, and feedings to ensure availability.
• Ensure patient needs are met per physician instructions.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
