Job summary

Adoration Health in Dyer, TN is seeking a Private Duty Licensed Practical Nurse for part-time daytime shifts (7:30am – 3:30pm) to provide 1:1 patient care per the client’s plan of care.

You will administer medications, document nursing actions, coordinate supplies and communicate with families, case managers and physicians to support safe, dignified home health care. Qualifications

• LPN/LPT/LVN license with current unrestricted registration in TN
• Valid driver’s license with proof of insurance
• CPR certification
• 1+ years nursing experience preferred
• Home health care experience preferred

Responsibilities

• Providing 1:1 care per client plan of care
• Administer medications, feedings, and other treatments as needed
• Document nursing actions and communicate changes to family and physicians
• Coordinate supplies, medications, and equipment to ensure adequate stock
• Ensure patient needs are met per physician instructions

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

Qualifications:

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
