Job summary

Adoration Health in Jackson, TN is seeking a dedicated Private Duty LPN to provide 1:1 patient care in home settings. This full-time, days-shift role supports patients under an individualized plan of care.

You will collaborate with families and case managers, administer medications and treatments, document care activities, and coordinate supplies. A valid TN license, CPR, and prior home health experience are preferred.

Qualifications

• LPN/LPT/LVN degree with active license in TN
• Valid TN driver’s license and proof of auto insurance
• CPR certification
• 1+ years nursing experience preferred
• Home health care experience preferred

Responsibilities

• Providing 1:1 care for client following plan of care at all times
• Administering medications, feedings, oxygen, ostomy care and assess results
• Documenting nursing actions and communicating with family, case manager and physicians
• Coordinating or ordering supplies, medications and feedings as needed
• Ensuring patient needs are met per physician instructions

Schedule: Full-Time; Days

Alternating week schedule

• Week 1: Monday, Tuesday, Thursday, Friday 7:30am - 5:30pm
• Week 2: Monday, Tuesday, Saturday, Sunday 7:30am - 5:30pm

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

Qualifications:

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
