Role Overview: The Licensed Practical Nurse (LPN) Navigator guides patient populations through complex care pathways, focusing on access, referral accuracy, sequencing of care, and continuity across services.

• Responsibilities: Review and triage referrals, collaborate with healthcare providers, communicate with patients, coordinate patient activities, support internal referral retention, provide patient and family education, maintain follow-up systems, serve as a liaison with scheduling, develop screening tools, investigate patient concerns, and monitor patient progress from referral to care completion.
• Qualifications & Skills: Minimum of 2 years of LPN experience, graduate from specialty nursing training programs, clinical knowledge for care facilitation, effective communication, strong coordination, and systems management abilities.
• Key Duties: Facilitate patient introduction to programs, ensure referral appropriateness, coordinate care plans, serve as a point of contact, promote efficient navigational procedures, and track patient status to improve care quality and access.
