Job Summary

The Licensed Practical Nurse (LPN) will serve as a patient navigator, guiding incoming calls, facilitating new patient entry, providing education, and supporting ongoing care. The role involves teamwork, clinical support, and coordination with healthcare providers to assist in diagnosis and treatment.

Responsibilities

1. Utilize clinical knowledge to facilitate patient introduction to the program.
2. Evaluate patient needs via telephone or documentation review.
3. Collaborate with healthcare providers on care plans.
4. Communicate with patients through mail, phone, and in person.
5. Ensure pertinent patient information is available for providers.
6. Coordinate patient needs and activities, including support groups and educational series.
7. Provide patient and family education as needed.
8. Maintain systems for follow-up and surveillance.
9. Coordinate scheduling and develop patient screening tools.
10. Investigate patient concerns related to appointments, communication, and billing.
11. Maintain clinical supplies and conduct medical record reviews.
12. Assist physicians and perform nursing care in clinical settings.
13. Inspect and maintain equipment and treatment rooms.

Qualifications

• Graduate from Specialty Training Program in Nursing
• Minimum of 2 years of LPN experience
• Licensed Practical Nurse certification
