Coverage Area

Scott, Jefferson, Floyd, Harrison and Crawford counties

Schedule

Full‑time, Monday through Friday, 8 a.m.–5 p.m.

Responsibilities

• Provide 1:1 care for clients following their plan of care while preserving dignity and independence
• Administer medications, feedings, oxygen, and ostomy care as needed per plan of care and assessment results
• Accurately document nursing actions, patient communication, and progress toward goals
• Coordinate or order supplies, medications, and feedings to ensure adequate availability
• Ensure all patient needs are met in accordance with physician instructions

Qualifications

• Licensed Practical Nurse (LPN), Licensed Practical Technician (LPT), or Licensed Vocational Nurse (LVN) from an accredited college with current, unrestricted registration in the state of practice and state of residence
• Valid driver’s license, acceptable driving record, and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home health care experience preferred
