Job summary

CarePlus Home Health is seeking a part-time Licensed Practical Nurse in Indianapolis to provide skilled home health care. You will follow nursing best practices, document accurately, and collaborate with the care team to support patients and families in their homes.

Applicants should have an Indiana LPN license, 2+ years preferred in home health, access to a car with valid insurance, and strong communication skills. We offer benefits and a supportive environment.

In This Role You Will:

The Licensed Practical Nurse- Home Health

Provide safe, high-quality home health care.

Follow nursing best practices and agency policies.

Communicate effectively with clients, families, and the care team.

Educate clients, families, and staff as needed.

Use equipment and supplies efficiently.

Complete documentation accurately and on time.

Support professional growth and teamwork.

This part-time LPN role offers a flexible schedule with a few shifts per week. The LPN will provide nursing care to patients, support their health needs, communicate with families and the care team, and complete required documentation. The position also includes participation in an on-call rotation for occasional clinical needs.

Who You Are:

• Graduate of an approved school of practical nursing, currently licensed as a Licensed Practical Nurse in the State of Indiana
• Two (2) years of experience in home health care or related health field (Preferred)
• Car with required proof of insurance coverage and an Indiana Driver’s license
• Knowledge of all types of home health equipment utilized by patients as well as forms and paperwork necessary for documentation
