Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a licensed practical nurse for home health in Somerset, KY. In this full-time role, you will provide 1:1 patient care in the home, following each patient’s plan of care with dignity and independence.

You’ll administer medications, monitor progress, document actions, and coordinate supplies. A valid KY LPN license, CPR certification, and a driver’s license are required, with 1+ year of nursing and home health experience

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in KY.
• Valid driver's license and proof of insurance.
• CPR certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Providing 1:1 client care per plan of care.
• Administering medications, feeding, oxygen, ostomy care as needed.
• Documenting nursing actions and communicating with physicians, families, and case managers.
• Coordinating or ordering supplies and medications to ensure availability.
• Ensuring patient needs align with physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
