Job summary

Amedisys in Stockbridge, GA seeks a licensed practical nurse to deliver compassionate home health care. You will verify plans with RNs, coordinate with physicians, teach patients and caregivers, monitor vitals and med reconciliation, and assist with procedures in patients' homes. Qualifications

• One year of clinical LPN experience.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met and report changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform physician-ordered procedures (e.g., Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
