Job summary

Amedisys is seeking a Licensed Practical Nurse to provide home health and hospice care in Florida, including Port Charlotte and surrounding communities. Join a leading home health provider with a focus on patient comfort and healing at home, offering flexible schedules and mission-driven work.

Responsibilities include verifying plans of care with RNs, patient assessment, medication review, vital signs, and education for patients and families.

Qualifications

• Minimum one year of clinical LPN experience.
• Current LPN license (state(s) of assignment).
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation.
• Perform physician-ordered procedures (e.g., Foley catheter change).
• Prepare equipment and materials using aseptic technique.
• Complete documentation timely and accurately.
