Job summary

Amedisys is seeking a licensed practical nurse for full-time day shifts in Georgia to deliver home-based care. Join one of the largest home health and hospice providers with flexibility, purpose and growth opportunities, right at patients' homes.

Expect to verify plans of care with RNs, collaborate with physicians, teach patients self-care, perform procedures, and document accurately to maintain high-quality care across visits.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state-specific.
• Current CPR certification.
• Valid driver's license and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs and review medications with reconciliation.
• Perform physician-ordered procedures (e.g., Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
