Job summary

Adoration Health is seeking a dedicated Home Health LPN to join our Huntsville, AL team. This full-time role provides 1:1 patient care in the home setting, administering medications and treatments per the care plan while maintaining patient dignity and independence.

The LPN must hold an active LPN/LPT/LVN license, have CPR certification, and a valid driver's license. Home health experience and 1+ years of nursing experience are preferred, with opportunities for growth and stability in a growing

• LPN/LPT/LVN license from an accredited nursing program with active registration in the practiced state.
• Valid driver's license and proof of car insurance.
• CPR certification required.
• 1+ years nursing experience preferred; home health care experience preferred.

Responsibilities

• Provide 1:1 client care according to the plan of care, preserving client dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as ordered and document results.
• Document nursing actions, communicate with family and physicians, and report changes in patient condition.
• Coordinate supplies, medications, and feedings to ensure adequate stock.
• Meet patient needs per physician instructions.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
