Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health LPN for Mobile, AL. The role focuses on delivering expert, patient-centered 1:1 home health care, with opportunities for professional growth and stability in a growing company.

The position is full-time, with typical weekday scheduling. Responsibilities include administering medications, documenting care, coordinating supplies, and maintaining communication with families and physicians.

Qualifications

• 1+ years of nursing experience preferred.
• Home Health care experience preferred.
• LPN/LPN license with current registration in state of practice.

Responsabilités

• Providing 1:1 care for client following plan of care at all times while maintaining client dignity and independence.
• Administering medications, feedings, oxygen, ostomy care as needed per plan of care and assessments of results.
• Document nursing actions of all care given and communicate with family, case manager, and physicians; inform changes in condition and needs to physician and families.
• Coordinating or ordering supplies, medications, and feedings to ensure adequate amounts are on hand.
• Ensure all patient needs are met per physician instructions.

Office Location: Mobile, AL Coverage area: Baldwin County

Schedule: Full-Time; Monday - Friday

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
