Job summary

Seasons Home Health Care seeks a dedicated Home Health Licensed Practical Nurse to join our clinician-owned team in Michigan's East-of-Rt131 service area. You will deliver personalized in-home care, monitor patient conditions, and collaborate with physicians to optimize treatment plans.

We value autonomy, flexibility, and a strong patient-first approach. Full-time role with comprehensive benefits, PTO, and a generous 401(k). Must have active MI LPN license, CPR, and reliable transportation.

Qualifications

• Active Michigan LPN license in good standing.
• Current CPR certification.
• Reliable transportation with valid auto insurance.
• Solid computer skills and ability to navigate an EMR system (WellSky a plus).
• Working knowledge of Medicare/Medicaid guidelines for Home Health.
• Strong independent work ethic and compassionate patient care.

Responsibilities

• Deliver personalized plans of care directed by physicians for homebound patients.
• Actively monitor condition changes to adjust care plans and reduce hospital visits.
• Communicate with physicians regarding medications and care adjustments.
• Teach and counsel patients and families on managing injuries and recovery.
• Complete patient records efficiently in the EMR system.
• Attend orientation and ongoing staff meetings for professional growth.

What You'll Do (Your Role & Impact)

• Prevent Re-Hospitalizations: Actively monitor condition changes to adjust care plans early and minimize hospital visits.
• Collaborate on Care: Communicate directly with physicians regarding medication needs and ongoing care adjustments.
• Empower Families: Teach and counsel patients and family members on managing injuries, diseases, and recovery processes.
• Digital Documentation: Complete patient care records efficiently in our WellSky EMR system.
• Continuous Growth: Attend orientation, ongoing staff meetings, and earn CEUs.

Requirements

What We’re Looking For

• Active Michigan LPN License (in good standing)
• Current CPR Certification
• Reliable Transportation & current auto insurance
• Tech Comfort: Solid computer skills and ability to navigate an EMR system (WellSky experience is a plus!)
