Job summary

Centria Healthcare is seeking a compassionate LPN or RN to provide one‑on‑one home health care for a client in Whitmore Lake, Michigan. You will support breathing, nutrition, transfers, and coordination with the family for weekly PT sessions.

Qualifications

• Active RN or LPN license in Michigan.
• Experience in home health, private duty, or long‑term care preferred.

Responsibilities

• Provide skilled nursing care (trach/vent care, medication administration, catheter care, etc.).
• Monitor and document vitals, symptoms, and changes in condition.
• Assist with ADLs such as repositioning, hygiene, mobility, etc.
• Support patient well‑being with compassionate and attentive care.

A Day in the Life

Your Day Will Include

• Respiratory Support: Keeping his lungs clear using a nebulizer and a cough assist machine three times a day.
• Nutrition: Managing his feeding tube and ensuring his daily intake is on track.
• Safe Movement: Using a ceiling lift for transfers—allowing you to move the client safely and comfortably without physical strain.
• Physical Therapy: Coordinating with the family to get him ready for his weekly PT appointments.
