Job summary

Amedisys, a leading home health and hospice provider, is seeking a licensed practical nurse for full-time field visits. You will work in patients' homes, verify plans of care with RNs, and coordinate with physicians to meet evolving needs.

The role emphasizes patient teaching, vital signs assessment, medication reconciliation, and documenting care promptly to support healing in a comfortable home setting.

Qualifications

• One year of clinical LPN experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Current LPN license valid for the state(s) assigned.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
