Job summary

Mallard Bay Nursing + Rehab is seeking Licensed Practical Nurses to join our team and deliver compassionate, patient-centered care to our residents. You will promote comfort, independence, and well-being while collaborating with a supportive team in a fast-paced skilled nursing environment.

As an LPN, you will supervise geriatric nursing assistants, administer prescribed medications, monitor residents for changes, and participate in admissions, discharges, and transfers.

Qualifications

• Current CPR certification.
• Current unrestricted LPN license in practicing state.

Responsibilities

• Provide direct patient care to residents promoting comfort and independence.
• Direct the daily activities of geriatric nursing assistants per regulations and facility policies.
• Assess changes in residents’ status and communicate with physicians and families; document accordingly.
• Transcribe physician orders and administer medications as ordered.
• Perform wound treatments and document observations as indicated.
• Assist with admissions, discharges, and transfers as required.

Licensed Practical Nurse (LPN) Requirements & Responsibilities

• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• Directs the daily activities of the geriatric nursing assistants by current federal, state, and local regulations and guidelines, and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Participates in the admission, discharge, and transfers of residents as required.
• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.
• Transcribes physician orders to the medical record and carries out orders as written.
• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.
• Performs wound treatments as per physicians’ orders, observes for changes, and documents accordingly as indicated.
