Job summary

Midland Care Connection is seeking a Licensed Practical Nurse to support participants in the PACE program, both in clinic and in community/home settings. The role emphasizes medication administration, health monitoring, and coordinated care within an interdisciplinary team.

You will travel throughout the community, maintain detailed records, reinforce care plans, and contribute to safer, more independent living for older adults and seriously ill individuals.

Qualifications

  • Graduate of an accredited Practical Nursing (LPN) program.
  • Current, unrestricted Licensed Practical Nurse (LPN) license in the State of Kansas
  • Current CPR/BLS certification or ability to obtain upon hire
  • Valid driver's license, reliable transportation, and an acceptable driving record for travel throughout the community
  • Minimum of one (1) year of nursing experience, preferably with older adults or chronic/complex conditions
  • Knowledge of basic nursing principles, medication administration, infection prevention, and chronic disease management
  • Strong assessment, observation, and communication skills with the ability to recognize and report changes in participant condition
  • Ability to work independently in participants' homes while collaborating effectively with an interdisciplinary team
  • Proficiency with electronic medical records (EMR) and basic computer applications
  • Ability to safely lift, transfer, and assist participants as needed

Responsibilities

  • Provide direct nursing support in the PACE center to deliver high-quality, coordinated care.
  • Administer medications and treatments; monitor health status and document care.
  • Communicate changes in condition to the care team; reinforce education from the team.
  • Prepare participants for appointments and maintain accurate medical records.

Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.

PACE Program The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.

Licensed Practical Nurse | Community & Clinic In the clinic, the LPN provides direct nursing support to help participants receive high-quality, coordinated care in the PACE center. Working under the direction of a registered nurse or provider and as part of the interdisciplinary team, the Clinic LPN assists with routine clinical care, administers medications and treatments, monitors participants' health status, documents care, and communicates changes in condition to the appropriate team members. This role also supports clinic operations by preparing participants for appointments, maintaining accurate medical records, reinforcing education provided by the care team, and helping ensure a safe, welcoming environment that promotes each participant's health, comfort, and independence. In the community, the LPN provides compassionate, hands-on nursing care to participants in their homes and throughout the community under the direction of a registered nurse. As part of the interdisciplinary care team, the Community LPN administers medications and treatments, performs routine nursing visits, monitors participants' health status, reports changes in condition, and reinforces individualized care plans. This role also educates participants and caregivers, supports chronic disease management, documents care accurately, and helps coordinate services to promote participant safety, independence, and quality of life while reducing avoidable hospitalizations.

Qualifications

Required

Preferred

  • Two or more years of experience in home health, hospice, geriatrics, long-term care, PACE, or community-based nursing
  • Experience caring for frail older adults with multiple chronic conditions
  • Knowledge of Medicare, Medicaid, and PACE regulations and standards
  • Experience with care coordination and interdisciplinary team collaboration
  • Experience using electronic health record systems and mobile documentation tools
  • Demonstrated ability to build trusting relationships with participants, caregivers, and community partners
