Silas Creek Rehabilitation Center in Winston-Salem, NC is seeking Licensed Practical Nurses to join a collaborative care team focused on resident outcomes and professional growth. The LPN will administer medications, collect vital signs, monitor resident condition, supervise CNAs, contribute to care planning, and maintain accurate documentation.

Eligible candidates must graduate from an accredited LPN program, hold an active NC LPN license, CPR, and prior long-term care experience is preferred;

Position Summary

The LPN provides nursing care and treatment in accordance with physician orders and established standards of practice. You’ll work closely with physicians, RNs, CNAs, and the interdisciplinary team to ensure excellent resident outcomes and a supportive team environment.
