Job summary

Rockville Center is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Skilled Nursing Facility setting.

The role focuses on geriatric resident care, medication administration (including IVs), vital signs, and family education. Candidates must hold a NY LPN license and be able to work alternating weekends, with supervision of CNAs as needed.

Qualifications

• Current NY LPN license required.
• Experience in geriatric/long-term care is a plus.
• Medication administration including IVs.
• Must be able to work with minimal supervision and supervise CNAs.

Responsibilities

• Provide care for residents in SNF and Long Term Care units.
• Administer medications and monitor outcomes.
• Take and record vital signs accurately.
• Provide resident and family education.
• Communicate with physicians, NPs, and other care team members.
• Supervise CNAs and assist with care planning.

Rockville Center is searching for a talented and motivated Licensed Practical Nurse (LPN) to join our dynamic team.

The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, must contain a license in the state of NY, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience.

LPN (Licensed Practical Nurse) Job Description:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
