Brothers of Mercy Nursing & Rehabilitation Center in Clarence, NY is seeking a licensed practical nurse (LPN) to join our mission-driven team caring for older adults in a compassionate, family-oriented environment.

We offer two care settings: Sacred Heart Home Assisted Living and the skilled nursing and rehab center, with opportunities to lead, coordinate care, administer medications, and collaborate with CNAs, therapists, and physicians.

Qualifications

• Current NY State LPN License is required.
• Positive, dependable, and caring with a team-oriented mindset.
• Ability to provide high-quality care while following established procedures and safety standards.
• Strong communication, documentation, and leadership skills.

Responsabilités

• Coordinate resident care and monitor overall wellness.

In this setting, LPNs take on a leadership-focused, care coordination role where relationships and oversight are key, while staying actively involved in the day-to-day care of residents.

What you’ll do:

• Oversee and participate in medication administration and treatments
• Respond to changes in condition, incidents, and daily needs on the floor
• Serve as a coach and supervisor for HHAs, PCAs, and Medication Technicians
• Communicate with families and the interdisciplinary team to ensure seamless care and follow-through
• Support a home-like environment where independence and dignity are prioritized
