Job summary

Titusville Nursing and Rehab is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Titusville, PA. You will provide patient-centered care to residents and support the nursing team in delivering high-quality services.

Responsibilities include medication administration, vital signs, ADLs, and accurate documentation, with collaboration across disciplines to implement care plans and pursue quality improvement initiatives.

Qualifications

• Current LPN license in PA (active).
• High school diploma or equivalent required.
• Experience in a long-term care or rehabilitation setting preferred.
