Titusville Nursing and Rehab is seeking an evening shift Licensed Practical Nurse (LPN) to provide high-quality patient care in our skilled nursing facility, collaborating with interdisciplinary teams to support residents and families.

About the Role:

• Provide direct patient care to residents in our skilled nursing facility, including administering medications, documenting patient records, and developing individualized care plans.

• Conduct routine assessments and evaluations to identify patient needs and develop effective care strategies.

• Collaborate with registered nurses, doctors, and other healthcare professionals to ensure seamless resident care.

• Maintain accurate and up-to-date records, reports, and charts.

• Demonstrate a commitment to patient-centered care, upholding the highest standards of quality, confidentiality, and respect.

Responsibilities:

• Provide skilled nursing care to residents, including wound care, medication administration, and respiratory therapy.
