Job summary

Bethany Center For Rehabilitation and Healing in Tennessee is seeking an LPN to join our post-acute care team. You will administer medications, assist with admissions and discharges, and document care in resident records.

This full-time or part-time position offers flexible shifts (Day/Night) and opportunities to work with a multidisciplinary team while upholding facility policies and standards.

Qualifications

• Nursing degree from an accredited college or university or graduate of an approved LPN nursing program.
• Infection control and facility policies adherence required.
• Post-acute care experience preferred.

Responsibilities

• Administer medications and provide treatments.
• Assist with admissions, transfers and discharges.
• Document resident care in medical records and logs.
• Educate residents and families about treatment and status changes.
• Adhere to infection control and DOH standards.
• Consult with physicians on plans of care and changes.
• Maintain resident confidentiality and privacy.
• Collaborate with interdisciplinary team to resolve issues.

Available Shifts: Day shift, Night Shift

Full Time and Part Time positions available.

Licensed Practical Nurse (LPN) Duties Include:

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents
• Document resident care services by charting in resident medical record and department records.
• Educate residents regarding treatment, rehabilitation, change of status, etc… as well as family members.
• Adhering to infection control policies and procedures as well as all other facility policies and procedures.
• Consult with resident’s physician regarding resident’s plan of care as well as notifying them of any changes.
• Maintain resident’s confidentiality and privacy.
• Assure quality of care by adhering to DOH standards of practice and facility standards of care.
• Assist with solving resident’s problems by working with the interdisciplinary team.

REQUIREMENTS/QUALIFICATIONS:

The Nurse must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN nursing program.
