Bethany Center For Rehabilitation and Healing is seeking a Licensed Practical Nurse (LPN) to join our post-acute care team. Positions are available for day and night shifts, with opportunities for full-time or part-time schedules and weekly/daily pay options.

The role includes medication administration, care documentation, resident education, and collaboration with physicians and an interdisciplinary team to uphold high standards of patient care.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN nursing program.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents.
• Document resident care services by charting in resident medical records.
• Educate residents and family members about treatment and status changes.
• Adhere to infection control policies and facility procedures.
• Consult with physicians regarding plan of care and changes.
• Maintain resident confidentiality and privacy.
• Ensure quality of care by following DOH and facility standards.
• Collaborate with interdisciplinary team to resolve resident problems.

Available Shifts: Day shift, Night Shift

Full Time and Part Time positions available.

Licensed Practical Nurse (LPN) Duties Include:

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents
• Document resident care services by charting in resident medical record and department records.
• Educate residents regarding treatment, rehabilitation, change of status, etc… as well as family members.
• Adhering to infection control policies and procedures as well as all other facility policies and procedures.
• Consult with resident’s physician regarding resident’s plan of care as well as notifying them of any changes.
• Maintain resident’s confidentiality and privacy.
• Assure quality of care by adhering to DOH standards of practice and facility standards of care.
• Assist with solving resident’s problems by working with the interdisciplinary team.

REQUIREMENTS/QUALIFICATIONS:

The Nurse must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN nursing program.
