Charter Senior Living in Sylvania, OH is seeking an LPN / Licensed Practical Nurse to join our assisted living and memory care teams. The role emphasizes resident-centered care, medication management, assessments, care planning support, and collaboration with families and physicians.

Qualifications

• Active LPN / Licensed Practical Nurse license in good standing.
• Assisted living, memory care, long-term care, or senior living experience preferred.
• Strong medication management and clinical assessment skills.
• Excellent communication and teamwork abilities.
• Compassionate and resident-centered approach to care.
• Ability to work in a fast-paced environment while maintaining attention to detail.
• Strong organizational and documentation skills.

Responsibilities

• Provide nursing care and clinical support to residents.
• Assist with resident assessments and care planning processes.
• Administer medications and treatments according to physician orders and company policies.
• Monitor resident conditions and communicate changes appropriately.
• Maintain accurate clinical documentation and medication records.
• Partner with caregivers, medication technicians, physicians, and families to ensure quality resident care.
• Support resident dignity, comfort, and quality of life.
• Assist with compliance, infection control, and quality assurance processes.
• Support and mentor caregiving staff as needed.

Position Summary

The LPN / Licensed Practical Nurse plays a vital role in supporting the health, wellness, and quality of life of residents within our Assisted Living and/or Memory Care communities. This position is responsible for resident assessments, care planning support, medication and treatment management, clinical documentation, and ongoing communication with residents, families, physicians, and caregiving teams.
