Charter Senior Living in Gallatin, TN is seeking an LPN to join our assisted living and memory care teams. You will provide nursing care, support assessments, administer medications, and collaborate with physicians and families to maintain resident well-being.

Qualifications

• Active LPN license in good standing.
• Assisted living, memory care, long-term care, or senior living experience preferred.
• Strong medication management and clinical assessment skills.
• Excellent communication and teamwork abilities.
• Compassionate and resident-centered approach to care.
• Ability to work in a fast-paced environment with attention to detail.
• Strong organizational and documentation skills.

Responsibilities

Position Summary

The LPN / Licensed Practical Nurse plays a vital role in supporting the health, wellness, and quality of life of residents within our Assisted Living and/or Memory Care communities.

This position is responsible for resident assessments, care planning support, medication and treatment management, clinical documentation, and ongoing communication with residents, families, physicians, and caregiving teams.
