Job summary

Apple Rehab West Haven is seeking a dedicated LPN Charge Nurse to lead nursing services for assigned residents, ensuring high-quality care and adherence to policies. The role focuses on medication administration, documentation, and collaboration with physicians, families, and care teams.

Starting pay ranges from $30-$37 per hour based on experience. Available shifts include 7a-3p, 3p-11p, and 11p-7a, with per diems welcome. Weekend on-call is required for all LPN positions.

Qualifications

• Valid state nursing license in good standing (or in the process of becoming a registered nurse).
• In-depth knowledge of nursing practices, medical procedures, and long-term care regulations.
• Strong attention to detail, excellent organizational skills, and the ability to manage multiple tasks effectively.
• Collaborative team player with previous experience in long-term care preferred.

Responsibilities

• Plans, executes, and oversees nursing services for assigned residents in accordance with quality care standards and physician instructions.
• Maintains professionalism in interactions with residents, physicians, families, peers, and support staff.
• Documents crucial information related to resident care accurately and comprehensively.

The LPN Charge Nurse plays a pivotal role in planning, executing, and overseeing nursing services for assigned residents. This includes adhering to quality care standards, facility policies, legal requirements, medical practices, and physician instructions

• Maintains professionalism in interactions with residents, physicians, families, peers, and support staff.
• Documents crucial information related to resident care accurately and comprehensively.
• Administers prescribed medications and oversees accurate record-keeping in line with facility policies.

Requirements:

Additional Expectations: All LPN positions require weekend on-call availability to support both staff and residents. Compensation details available upon inquiry. Per diem staff are expected to fulfill (2) weekend shifts monthly to meet requirements.
