Garden Terrace Alzheimer's Center of Excellence, affiliated with Life Care Centers of America, is seeking an experienced LPN to join a dedicated memory-care team in Kansas. You will provide skilled nursing care for residents with dementia, monitor conditions, and collaborate with CNAs and therapists to maintain high-quality daily care.

The role emphasizes compassionate, patient-centered services within a supportive, multidisciplinary environment.

Qualifications

• Graduated from an accredited LPN program.
• Active LPN/LVN license in good standing.
• CPR certification required or obtain during orientation.
• One year skilled nursing experience preferred.

Responsibilities

• Monitor and report changes in patient condition promptly.
• Assign duties, instruct, and provide clinical supervision to CNAs.
• Deliver quality nursing care with a patient-centered approach.

As an important member of our interdisciplinary team, the LPN delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and Life Care standards. Your role will involve monitoring and reporting changes in the patient’s condition. On a daily basis you must be able to assign duties, instruct, and provide clinical supervision to CNAs.

Education, Experience, And Licensure Requirements

• Graduated from an accredited LPN program
• Currently licensed/registered in applicable State. Must maintain an active Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing throughout employment.
• One (1) year skilled nursing experience preferred
• CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
