Job summary

Rockford Health Care Center in Ohio is seeking compassionate Licensed Practical Nurses (LPNs) to join a dedicated care team under new management. You will perform assessments, admission duties, discharges, wound care, and lead CNAs, while maintaining high standards of patient care.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
