Job summary

Rockford Health Care Center in Kentucky is seeking compassionate LPNs to provide high-quality nursing care for residents under new management.

You will conduct assessments, admit patients, manage discharges, and lead CNAs/CMTs while adhering to facility protocols and safety standards.

Responsibilities

• Manage discharges: complete docs, notify physician/guardian, arrange transport.
• Provide wound care: follow protocols, irrigate, pack, dressings.
• Lead CNAs and CMTs, ensure facility protocol compliance.
• Respond effectively to emergency situations.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
