Clinical Documentation: Maintain accurate and thorough electronic records of all resident care, observations, and treatments.
Order Management: Precisely transcribe physician orders to MARs and promptly notify the pharmacy of changes to ensure seamless care.
Medication Administration: Prepare and safely administer medications as ordered, strictly adhering to facility protocols.
Resident Monitoring: Proactively observe residents for changes in condition, take vital signs, and elevate concerns to the RN or Unit Manager.
Treatment Implementation: Skillfully perform and document wound care and clinical procedures within your licensed scope of practice.
Unit Supervision: Provide clinical guidance and direction to CNAs to ensure high standards of direct care delivery.
Licensure: Must possess an active and unrestricted Licensed Practical Nurse (LPN) License in the state of Wisconsin.
Collaboration: Demonstrated ability to work effectively within a multidisciplinary clinical team.
Experience: Previous experience in long‑term care or skilled nursing is preferred, but highly motivated new licensees are encouraged to apply!
