Job summary

Complete Care Management in Wisconsin is seeking a dedicated Licensed Practical Nurse (LPN) for full-time 2nd shift to support our clinical team and residents.

You will document care, manage orders, administer medications, monitor residents, and guide CNAs, with a focus on quality, compassion, and timely communication within a supportive environment.

Qualifications

• Active LPN license in Wisconsin.
• Experience in long-term care or skilled nursing preferred.
• Ability to collaborate within a multidisciplinary team.
• New licensees encouraged to apply.

Responsibilities

• Clinical Documentation: Maintain accurate and thorough electronic records of all resident care, observations, and treatments.
• Order Management: Transcribe physician orders to MARs and notify pharmacy of changes.
• Medication Administration: Prepare and administer medications per protocols.
• Resident Monitoring: Observe residents for changes, take vitals, escalate to RN/Unit Manager.
• Treatment Implementation: Perform wound care and clinical procedures within scope.
• Unit Supervision: Guide CNAs to ensure high standards of care.

• Treatment Implementation: Skillfully perform and document wound care and clinical procedures within your licensed scope of practice.

• Unit Supervision: Provide clinical guidance and direction to CNAs to ensure high standards of direct care delivery.

Qualifications

• Licensure: Must possess an active and unrestricted Licensed Practical Nurse (LPN) License in the state of Wisconsin.

• Collaboration: Demonstrated ability to work effectively within a multidisciplinary clinical team.

• Experience: Previous experience in long-term care or skilled nursing is preferred, but highly motivated new licensees are encouraged to apply!
