Complete Care at Maple Grove in Madison, WI is seeking a dedicated Licensed Practical Nurse (LPN) for full-time 2nd shift. You will document resident care, transcribe orders, administer medications, monitor residents, and assist with wound care and procedures within your licensed scope.

Qualifications

Responsibilities

• Maintain accurate electronic documentation of resident care.
• Transcribe physician orders to MARs and notify pharmacy of changes.
• Prepare and administer medications according to facility protocols.
• Monitor residents, take vitals, escalate concerns to the RN/Unit Manager.
• Perform wound care and other clinical procedures within licensed scope.
• Provide guidance to CNAs to ensure high-quality direct care.

Core Responsibilities

• Clinical Documentation: Maintain accurate and thorough electronic records of all resident care, observations, and treatments.
• Order Management: Precisely transcribe physician orders to MARs and promptly notify the pharmacy of changes to ensure seamless care.
• Medication Administration: Prepare and safely administer medications as ordered, strictly adhering to facility protocols.
• Resident Monitoring: Proactively observe residents for changes in condition, take vital signs, and elevate concerns to the RN or Unit Manager.
• Treatment Implementation: Skillfully perform and document wound care and clinical procedures within your licensed scope of practice.
• Unit Supervision: Provide clinical guidance and direction to CNAs to ensure high standards of direct care delivery.

Qualifications

• Licensure: Must possess an active and unrestricted Licensed Practical Nurse (LPN) License in the state of Wisconsin.

• Experience: Previous experience in long-term care or skilled nursing is preferred, but highly motivated new licensees are encouraged to apply.
