Job summary

Nazareth Home in Louisville, KY is seeking a Licensed Practical Nurse (LPN) to join our team on night shifts. We offer a low nurse‑to‑resident ratio, a dedicated nursing team, and hands‑on care focused on building meaningful relationships with residents.

Responsibilities include administering medications and treatments, documenting care, communicating with families and the care team, and supporting nursing assistants to ensure safe, high‑quality care in a long‑term care setting.

Qualifications

• Graduate of an accredited school of nursing
• Licensed by the Kentucky Board of Nursing as an LPN
• Current CPR certification

• Provide preventative, therapeutic, and rehabilitative nursing care
• Administer medications and treatments and monitor resident responses
• Observe and report changes in condition to the care team
• Maintain accurate clinical documentation and medication records
• Communicate with residents, families, physicians, and team members
• Support nursing assistants and promote teamwork on the unit
• Respond calmly and effectively to emergencies

Education
