Birmingham Rehabilitation and Wellness Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary rehab team in Forestdale, AL. You will provide direct patient care under RN supervision, administer medications, and support in wound care while collaborating with therapists to achieve rehab goals.

Responsabilidades

• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and performing vital signs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Maintain accurate and up-to-date patient records and documentation
• Participate in patient education and discharge planning to ensure a smooth transition to home or community-based care
• Collaborate with physical, occupational, and speech therapists to ensure comprehensive patient rehabilitation
• Maintain a safe and clean environment for patients, staff, and visitors

Shifts Available: Monday-Friday 2:30pm-11:00pm/Weekends 6:30pm-7:00am with baylor plan

Job Summary:

Birmingham Rehabilitation and Wellness Center is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As an integral part of our interdisciplinary team, the LPN will provide high-quality patient care and support in a dynamic and fast-paced rehabilitation setting.

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and performing vital signs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team

Requirements:
