Job summary
Birmingham Rehabilitation and Wellness Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our rehabilitation team in Forestdale, Alabama. The LPN will provide direct patient care under RN supervision, administer medications, perform wound care, and monitor vital signs in a fast-paced setting.
You will develop care plans with the interdisciplinary team, maintain accurate records, educate patients, assist with discharge planning, and collaborate with therapists.
Qualifications
- Current LPN licensure in the State of Alabama.
- CPR Certification.
Responsibilities
- Provide direct patient care under supervision of a registered nurse, including administering medications, wound care, and vital signs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Maintain accurate and up-to-date patient records and documentation.
- Participate in patient education and discharge planning to ensure a smooth transition to home or community-based care.
- Collaborate with physical, occupational, and speech therapists to ensure comprehensive patient rehabilitation.
- Maintain a safe and clean environment for patients, staff, and visitors.
