Job summary

Birmingham Rehabilitation and Wellness Center is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in Forestdale, AL. The LPN will provide high-quality patient care in a fast-paced rehabilitation setting under the supervision of a registered nurse.

Shifts available: Monday-Friday 2:30pm-11:00pm and weekends 6:30pm-7:00am with Baylor plan. Requirements include current Alabama LPN licensure and CPR certification.

Qualifications

• Current LPN licensure in the State of Alabama.
• CPR Certification.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and performing vital signs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Maintain accurate and up-to-date patient records and documentation.
• Participate in patient education and discharge planning to ensure a smooth transition to home or community-based care.
• Collaborate with physical, occupational, and speech therapists to ensure comprehensive patient rehabilitation.
• Maintain a safe and clean environment for patients, staff, and visitors.
